Occasionally, cases might present later on in adolescence with problems of sexual maturation. Our case report presents a true hermaphrodite with normal male phenotype that presented as a left testicular mass, two years after being diagnosed with Sertoli cell only syndrome in the contralateral testis. Histological examination of the left testis showed ovarian, fallopian tube, myometrial, endometrial, and epididymal tissue. This combination of findings is found in approximately one-third of true hermaphrodites, but it is very rare to present clinically as an inguinoscrotal mass.

Hermaphroditism , the condition of having both male and female reproductive organs. Hermaphroditic plants —most flowering plants, or angiosperms —are called monoecious, or bisexual. Hermaphroditic animals—mostly invertebrates such as worms , bryozoans moss animals , trematodes flukes , snails , slugs , and barnacles —are usually parasitic, slow-moving, or permanently attached to another animal or plant. In humans, conditions that involve discrepancies between external genitalia and internal reproductive organs are described by the term intersex.
True hermaphroditism , clinically known as ovotesticular disorder of sex development ,  is a medical term for an intersex condition in which an individual is born with ovarian and testicular tissue. More commonly one or both gonads is an ovotestis containing both types of tissue. Although similar in some ways to mixed gonadal dysgenesis , the conditions can be distinguished histologically. External genitalia are often ambiguous, the degree depending mainly on the amount of testosterone produced by the testicular tissue between 8 and 16 weeks of gestation.
Intersex is a group of conditions where there is a discrepancy between the external genitals and the internal genitals the testes and ovaries. The older term for this condition is hermaphroditism. Although the older terms are still included in this article for reference, they have been replaced by most experts, patients and families. Increasingly, this group of conditions is being called disorders of sex development DSDs. Each one is discussed in more detail below.
